Abstract

Objective: To analyze the changes of serum peroxisome proliferator-activated receptor gamma (PPAR-γ) mRNA and matrix metalloproteinase-9 (MMP-9) mRNA levels in evaluating the occurrence and progression of intracranial aneurysms. 

Methods: A total of 90 patients with intracranial aneurysms, who were admitted in the department of neurology of our hospital from April 2016 to April 2018, were recruited for the observation group. Based on findings of cranial CT or lumbar puncture, 31 patients with rupture aneurysm were placed in the rupture group and 59 patients with non-rupture aneurysm were assigned to the non-rupture group. In addition, 45 healthy individuals who got health checkup in our hospital were enrolled in the healthy group. The expressions of serum PPAR-γ mRNA and MMP-9 mRNA were detected by RT-PCR. The value of changes in PPAR-γ mRNA and MMP levels in assessing the development and progression of intracranial aneurysm was analyzed by an ROC curve. 

Results: Patients with a ruptured aneurysm had a much longer heavy alcohol drinking history than those with un-ruptured aneurysm or healthy individuals, and the difference was statistically significant (P<0.01). There was no significant difference in smoking, hypertension, hyperlipidemia and diabetes between the three groups (P>0.05). The expression level of PPAR-γ mRNA in patients with intracranial aneurysm rupture was significantly higher than that in the other two groups (P<0.01) and was much higher in the non-rupture group than that in the healthy group (P<0.01). The expression level of MMP- 9 mRNA was significantly lower in the rupture group than in the non-rupture and the healthy groups (P<0.01). The expression level of MMP-9 mRNA was significantly lower in the non-rupture group than that in the healthy group (P<0.01). The following values for the area under the ROC curve (AUC) obtained when using serum PPAR-γ mRNA and MMP-9 mRNA were obtained: 0.830 and 0.827, respectively, when diagnosing intracranial aneurysms; and 0.887 and 0.851, respectively, when diagnosing intracranial aneurysm rupture.  The sensitivities with PPAR-γ mRNA and MMP-9 mRNA were 84.8% and 82.9%, respectively, and the respective specificities were 80.9% and 78.7. Both indicators could be used as specific indicators to evaluate the development and progression of intracranial aneurysms. 

Conclusions: PPAR-γ mRNA expression is significantly increased and MMP-9 mRNA expression is greatly decreased in the serum of patients with intracranial aneurysm rupture. Both indicators play an important role in evaluating the occurrence and progression of intracranial aneurysms in clinic, which provides a theoretical basis for treating intracranial aneurysms rupture in practice.
