Abstract

Objective: To analyse and explore the value of serum cystatin C combined with microRNA-16-5p in the early diagnosis of diabetic kidney disease (DKD). 

Methods: A total of 132 patients with diabetes were selected from May 2017 to March 2018. Among these, 65 patients with diabetes were placed in group T and 67 patients with diabetic nephropathy were placed in group TS. The general data of the T and TS groups were compared (total cases, sex, age, weight). Moreover, the serum cystatin C level and mir-16-5p levels of the T group and TS group were compared, while the relationship between serum cystatin C and mir-16-5p and estimated glomerular filtration rate of diabetic nephropathy was analysed by Pearson linear correlation analysis. Furthermore, an ROC curve was used to study the diagnostic value of serum cystatin C and microRNA-16-5p in diabetic nephropathy. 

Results: The serum cystatin C level in the TS group was significantly higher than that of the T group (P<0.01). The level of microRNA-16-5p in the TS group was significantly lower than that of the T group (P<0.01). Moreover, serum cystatin C level was negatively correlated with eGFR in diabetic nephropathy (r=-0.804, P<0.01). The level of microRNA-16-5p was positively correlated with eGFR in diabetic nephropathy (r = 0.826, P<0.01). The area under the curve of serum cystatin C level for diagnosis of diabetic nephropathy was 0.831, the cut-off point was 2.35 mg/L, the sensitivity was 78.35% and the specificity was 92.17%. Furthermore, the ROC curve of serum cystatin C level for the diagnosis of diabetic nephropathy was 0.897, while the critical value was 0.671 mg/L, the sensitivity was 84.92% and the specificity was 90.54%. The area under line was 0.945, while the sensitivity was 87.28% and the specificity was 92.38%. 

Conclusion: Serum cystatin C combined with microRNA-16-5p represents an important indicator in the early diagnosis of diabetic nephropathy and can provide some reference for clinical diagnosis.
